Michelle Bank, NP
Certified Nurse Practitioner
is a certified nurse practitioner who earned a Bachelor of Science in Nursing in 2012 from Maryville University and a Master of Science in Nursing in 2018 from Maryville University. She is also a certified Adult-Gerontology Nurse Practitioner. She joined the staff in 2023.
Ricardo Bolaños, MD
Obstetrician and Gynecologist
is board-certified in Obstetrics and Gynecology. He attended St. Louis University School of Medicine Obstetrics, Gynecology and Women’s Health in St. Louis, MO and Ponce School of Medicine in Puerto Rico. He joined the staff in 2014.
Jay Brieler, MD
Family Medicine
is a board-certified family medicine physician. Dr. Brieler graduated from Southern Illinois University School of Medicine in 1999 and completed his residency at Ramsey Family and Community Medicine Residency in 2003. He joined the staff in 2013. Dr. Brieler has been recognized as a Castle Connolly Top Doctor since 2021.

is board-certified in Pediatrics. She attended Washington University School of Medicine Department of Pediatrics in St. Louis, MO and New York Medical College. She joined the staff in 2014.

is a certified Family Nurse Practitioner who earned a Bachelor of Science in Nursing from the University of Central Florida in Aug. 2007. Daniel then earned a Master of Science in Nursing-AGNP from Maryville University in May 2017. He joined the staff in 2025.

is a board-certified family medicine physician. She attended the University of Missouri School of Medicine in Columbia, MO. Dr. Drallmeier completed her residency at the University of New Mexico Family Medicine Residency and Internship in June 2017. She joined the staff in July 2018.
Alex Gafford, DC
Chiropractor
is a licensed chiropractor. He earned his degree from Logan College of Chiropractic in 2004. Dr. Gafford is certified in meridian therapy/acupressure/acupuncture. He joined Family Care in 2020.
Mark Greenberg, MD
Pediatric Medicine
is a board-certified pediatrician who attended Northwestern University and obtained his medical degree from Ross University School of Medicine in 1984. He trained at Rush-Presbyterian-St. Luke’s Hospital in Chicago and Children’s Hospital of Wisconsin in Milwaukee. He has practiced in St. Louis, the Chicago area, and Phoenix for almost 40 years and is excited to be back at Family Care Health Centers, having rejoined the staff in June 2025.
Elizabeth Keegan-Garrett, MD, MPH
Family Medicine
is a board-certified family medicine physician. Dr. Garrett earned her Master’s Degree in Public Health from Harvard, earned her medical degree from Oregon Health & Science University, and completed her family medicine residency at the University of Missouri. She joined the staff in 2009.
Chyleigh Harmon, MD
Family Medicine
is a board-certified family medicine physician. Dr. Harmon earned her medical degree from Saint Louis University School of Medicine in May 2021. She completed her residency at Saint Louis University Family Medicine Residency in June 2024. She joined the staff in August 2024.
Miquia Henderson, MD
Pediatric Medicine
is a board-certified pediatrician who earned her medical degree from Washington University School of Medicine in St. Louis, MO. She completed her pediatric internship and residency at Washington University School of Medicine Department of Pediatrics – St. Louis Children’s Hospital – Barnes Jewish Hospital Consortium. Dr. Henderson joined the staff in March 2023.

is a certified women’s health nurse practitioner. She holds a Master of Science in Nursing from the University of Missouri-St. Louis and a Bachelor of Science in Nursing from Deaconess College of Nursing in St. Louis. She joined the staff in 2022.

is a licensed chiropractor. She earned her doctoral degree in chiropractic from Logan University in 2022 following her training in integrated care at Affinia Healthcare and the St. Louis VA. She joined the staff in 2023.
David Le, MSN, FNP-C

is a certified nurse practitioner who earned a Bachelor of Science in Nursing in May 2016 from Southern Illinois University, Edwardsville, IL and a Master of Science in Nursing-Family Nurse Practitioner in August 2021 from Maryville University in St. Louis, MO. He joined the staff in August 2023.
Joseph Markey, MD

is a board-certified internal medicine physician. He attended medical school at the McGovern Medical School at the University of Texas Health Sciences Center at Houston. Dr. Markey completed internal medicine residency at White River Health in Batesville, Arkansas in June 2025. He joined the staff in August 2025.

is a board-certified pediatrician who earned her medical degree from St. Louis University School of Medicine. She completed her pediatric residency at St. Louis University-Cardinal Glennon Children’s Hospital. Dr. McAtee joined the staff in 2008.

is a board-certified family medicine physician. She attended Saint Louis University School of Medicine. Dr. Meyr completed her residency at Saint Louis University Family Medicine Residency in June 2019. She joined the staff in July 2019.
Heidi Miller, MD
Internal Medicine
is a board-certified internal medicine physician who provides medical care for adults and adolescents. She attended Harvard Medical School and completed her residency at Harvard’s Brigham and Women’s Hospital in Boston, MA. Dr. Miller joined the staff in 2004.
Robert Poeppelmeier, DC
Chiropractor
is a licensed chiropractor. He earned his degree from Logan University in 2023. Dr. Poeppelmeier is certified in acupuncture and is fluent in Spanish. He joined Family Care in October 2023.
Carmen Purvis, MD
Family Medicine
is a board-certified family medicine physician. She earned her Medical Degree from The University of Texas Health and Science Center at Houston-McGovern Medical School in May 2022. Dr. Purvis completed her residency at Saint Louis University Family Medicine Residency in June 2023. She also completed a Maternal Child Health Fellowship at the University of Rochester School of Medicine and Dentistry in July 2024. She joined the staff in September 2024.
Susan Reynolds, MD
Family Medicine
s a board-certified family physician. She attended Washington University in Saint Louis as an undergraduate, Baylor College of Medicine in Houston for medical school, and returned to the Saint Louis area for residency at Lutheran Medical Center. She has practiced in the south city and metro-east areas for many years and started with Family Care Health Centers in April of 2025.
Kathryn Rudy, APRN, FNP-BC

is a board-certified family nurse practitioner who earned a Bachelor of Science in Nursing from Florida State University in April 2007 and a Master of Science in Nursing-Family Nurse Practitioner from the University of Missouri-St. Louis in August 2017. She joined the staff in July 2025.
Kenya Shotwell, MSN, APRN, FNP-BC
Certified Nurse Practitioner
is a certified family nurse practitioner who began her journey in nursing by completing an LPN program at St. Louis College of Health Careers in 1995. The journey continued as she completed an RN Bridge program at Lutheran School of Nursing in November 2004. Kenya went on to Webster University and completed a Bachelors of Science in Nursing in 2011. She completed her dream of being a Nurse Practitioner by finishing the Masters of Science in Nursing-Family Nurse Practitioner program at Maryville University in August 2015.
Abbe L. Sudvarg, MD

is a board-certified family physician. Dr. Sudvarg attended medical school at the University of Missouri-Columbia and completed her family practice residency training at Family & Community Medicine in Columbia, MO. She joined the staff in 1994. Dr. Sudvarg has been recognized as a Castle Connolly Top Doctor since 2021.

is a physician assistant who received her certifications and training at Bethel University in St. Paul, Minnesota. She joined FCHC in 2025.
Jacob Wallace, MD
Obstetrics and Gynecology
is a residency-trained OBGYN. He attended the Geisinger Commonwealth School of Medicine in Scranton, PA. Dr Wallace completed his residency at Saint Louis University in June 2025. He joined the staff in July 2025.
Jaime Zengotita, MD, MHA
Chief Medical Officer
is a Board-Certified Family Medicine Physician. He received his Bachelor of Biology at the University of Puerto Rico and completed his medical training in 1997. In June of 2000, he completed his family medicine residency at Summa Health Systems in Akron, OH, where he also served as Chief Resident. Dr. Zengotita completed an Executive Master’s program in Health Administration from Missouri State University in 2019. He joined FCHC in 2024.
Darryl Zinck, MD
Family Medicine
is a board certified family medicine physician. He earned his medical degree in May 1991 from the University of Missouri School of Medicine in Columbia, MO. He completed his residency in 1994 at the University of Wisconsin School of Medicine Department of Family Medicine in Madison, WI. He joined the staff in November 1995.

Priya Dey, DDS, MPH, DPH
Dentist
Dr. Dey practices as a general dentist. She earned her Doctor of Dental Surgery degree from Ajman University in the United Arab Emirates and completed her Master’s in Public Health at the University of Southern Mississippi. She also completed a Dental Public Health residency at Temple University in Philadelphia. Dr. Dey brings experience from private practice in Dubai and India, as well as two years service at an Federally Qualified Health Center in Northeast Vermont prior to joining the staff in 2025.
Carol D. Henley, DDS
Chief Dental Officer
is a dedicated public health dentist with more than 35 years of clinical and leadership experience. Dr. Henley earned her Doctor of Dental Surgery (DDS) from the University of Missouri–Kansas City School of Dentistry and her Bachelor of Arts in Biology from Benedictine College in Atchison, Kansas. Dr. Henley has been a passionate advocate for health equity, patient education, and mentoring the next generation of dental professionals. She has worked extensively with programs like “Give Kids a Smile,” the Missouri Primary Care Association, and Head Start initiatives in collaboration with the Urban League and Youth In Need. Dr. Henley is excited to help shape the future of dental care, with a vision to build strong oral health teams, expand access to preventive care, and ensure every patient—from infancy to older adulthood—receives the quality dental services they deserve. Dr. Henley joined the staff in November 2025.

practices as a dental hygienist and is our Dental Program Manager. Avery joined the staff in 2025.
Alyssa Ly, DDS
Dentist, Associate Dental Director
practices as a general dentist. She earned her Doctorate of Dental Surgery from the University of Missouri, Kansas City, MO in 2023. She has experience working as a dental assistant and a clinical extern. She joined the staff in August 2023.

is a licensed general dentist. He attended University of Washington School of Dentistry in Seattle, WA, where he obtained his Doctorate of Dental Surgery. Dr. Murphy joined the staff in July 2025.
Sapana Shrestha, DDS

practices as a general dentist. Dr. Shrestha graduated from the University of the Pacific Arthur A. Dugoni School of Dentistry in June 2018. She has experience working in private practice in St. Louis prior to joining the staff in 2019.

Jacquelyn Cattage, PhD

is a licensed psychologist. She earned her doctoral degree in Clinical Psychology in 2012 from St. Louis University after completing her internship training in child and adolescent psychology at Applewood Centers, Inc in Cleveland, OH. She joined the staff in 2013.
John Eads, LCSW
Behavioral Health Consultant
is a licensed master social worker. He earned his Master of Social Work in 2017 from the Brown School of Social Work at Washington University in St. Louis . He served in the Peace Corps (Peru, Youth Development) immediately after obtaining his Bachelor of Social Work from Murray State University. He joined the staff in January, 2025.
Rachel Gorin, LMSW
Care Manager
is a licensed master social worker in the state of Missouri. She received her bachelor’s degree in English from Maryville University and her masters in social work at the University of Nebraska – Omaha. She has been part of the team since November 2024.

Dianna Moses, PhD
Clinical Psychologist
s a licensed clinical psychologist who earned her doctorate at the University of Missouri in St. Louis and completed an internship and postdoctoral training in primary care settings of the US Air Force. Dr. Moses has worked in outpatient mental health clinics and private practice, providing clinical intervention and comprehensive psychological assessments for a variety of mental health conditions in children, adolescents, and adults. Dr Moses joined the staff in December 2024.
Carmen Murphy, LCSW
Care Manager
is a licensed clinical social worker in the state of Missouri. She earned her bachelor’s and master’s degrees in Social Work from Missouri State University in 2021 and 2022. She has worked as a school-therapist at Lutheran Family and Children Services of Missouri. She has experience working with both children and parents in the behavioral health field. She joined the staff in July 2025.
Lakshmi Naveen, MD
Adult Psychiatrist
Is a licensed psychiatrist. She earned her Bachelor’s degree of Medicine and Surgery from Rajiv Gandi University in 1998. After completing her residency at Saint Louis University (SLU), she then went on to complete a Fellowship of Geriatric Psychiatry at the department of Neurology and Psychiatry at SLU in 2018. In addition to her clinical experiences, she has research experience in an Epidemiology and Prevention research group at Washington University. She joined the staff in 2025.

is a licensed Clinical Psychologist. She earned her doctoral degree in Clinical Psychology from Saint Louis University in 2015 after completing her internship training in integrated healthcare at the Charlie Norwood VA in Augusta, Georgia. She joined the staff in 2016.
Rocky Sieben, LCSW

is a Licensed Clinical Social Worker. Rocky was born and raised in Nebraska. He has been a social worker for 36 years and enjoys teaching Mindfulness anywhere, anytime. Rocky enjoys meditation, Qigong exercises, taking walks, and riding his e-bike through different parks throughout St. Louis. Rocky joined the staff in May 2006.

Chief Behavioral Officer
is a Licensed Clinical Social Worker. She earned a bachelor’s degree in Social Work from Longwood College in Virginia and a Master of Social Work from Washington University in St. Louis. Celeste is trained in Relational-Cultural Therapy and is skilled in working with people who have survived complex trauma. Celeste has experience providing clinical social work care across a range of settings in Illinois, Massachusetts, and Missouri. She joined the staff in August 2025
