Family Care Health Centers (FCHC) wants to be your health care home. We are proud to be recognized by the NCQA as a Patient-Centered Medical Home (PCMH).
FCHC was created with the sole purpose of serving people who need care and were not receiving it. Fifty-five years later, patients are still at the forefront of care. A patient-centered medical home uses a model of care focused on patients and their individual needs. Research shows that PCMHs improve quality and the patient experience. Practices that earn recognition have made a commitment to continuous quality improvement and a patient-centered approach to care.

Family Care Health Centers uses a team-based approach to health care. Meaning:

- Your primary care provider is able to bring in a mental health specialist and/or a Registered Dietitian during your visit (no additional cost!).
- Your primary care provider talks with your mental health specialist, nutritional specialists, dentist, and/or optometrist regularly. Together, along with you, a patient care plan is established.

WHAT TO BRING TO YOUR FIRST APPOINTMENT
Please bring your insurance information (all insurance cards) and appropriate co-pay (at least $30.00 for sliding fee) and please arrive 30-45 minutes before your appointment time to complete the necessary paperwork.
