Our Departments
Family Care Health Centers (FCHC) has multiple departments for you to review and learn more about what you can bring to its teams.

FCHC is committed to providing convenient, comprehensive care, at costs affordable for its patients. Most private insurance plans are accepted, along with Medicare and Medicaid. Charges for patients without insurance are calculated on a sliding fee depending on income and family size.

FCHC provides dental care for all ages at both of our current locations. FCHC dentists perform limited oral surgery. We will provide a referral should a specialist be needed to meet a patient’s specific oral health needs.

FCHC’s behavioarl health services are person-centered, trauma-informed and integrated with primary care. The behavioral health team provides care to children and adults, psychiatric services, substance use disorder support, health coaching, and case management to pediatric and adult patients. These services are accessible to FCHC patients through referrals by their primary care provider.

FCHC is proud to offer our patients the convenience of getting their prescriptions on-site typically within 15-minutes of their appointment. We also offer FREE home prescription delivery services to ALL of our patients.
As a federally qualified health center, we have the ability to provide prescriptions at lower costs than other pharmacies.

FCHC’s Nutrition Services department provides three key services: medical nutrition therapy, lactation services and the Missouri WIC program (Women, Infants & Children).

Supervises: Shares supervision of providers at a specific site with Chief Medical Officer. Medical consultation to Nurse Practitioners and Physician Assistants of same specialty, ie. Pediatrician to PNP, Family Physician to PNP and OB/GYN Practitioners.
DEPARTMENT: MEDICAL SERVICES
JOB TITLE: ASSOCIATE MEDICAL DIRECTOR/PRIMARY CARE
PHYSICIAN: INTERAL MEDICINE, FAMILY PRACTICE, OB/GYN, PODIATRY, PEDIATRICIAN, AND GENERAL PRACTICE ASSOCIATE MEDICAL DIRECTOR/PRIMARY CARE PHYSICIAN
III. PRIMARY RESPONSIBILITIES:
1. Recruits, interviews, and recommends hiring of providers employed at a respective site to Chief Medical Officer.
2. Reviews and coordinates Continuing Medical Education (CME), annual leave, personal leave and unpaid leave. Makes recommendations to Chief Executive Officer for leave requiring Chief Executive Officer approval.
3. Coordinates clinical scheduling; shares development of on-call schedule and hospital rounding with Chief Medical Officer.
4. Directs disciplinary action, conflict resolution, and incident reporting involving providers. All incident reports must go to the Chief Medical Officer for signature prior to being forwarded to the Chief Executive Officer and CQI Committee.
5. Monitors and remedies provider productivity and patient satisfaction issues.
6. Provides orientation for new providers in conjunction with Chief Medical Officer and Human Resource Director.
7. Makes recommendations to Chief Medical Officer and Chief Executive Officer regarding provider contract renewals and terms of renewals, as well as performs performance reviews of team.
8. Ensures agency productivity, financial and patient satisfaction goals are attained.
9. Coordinates, facilitates and/or attends Department Head/Team Leader, provider, and site meetings..
10. Attends Board of Directors meetings at invitation of Chief Executive Officer.
11. Monitors providers’ clinical staffing to ensure adequate access for all patients during all hours of operation; works in a tandem with Operations Supervisor(s) and Chief Operations Officer to ensure unified agency effort in providing adequate access to services.
12. Serves as conduit between providers and Human Resource Director to ensure provider benefits are managed according to FCHC Personnel Policies.
13. Supports and/or participates in the discharge committee process of patients demonstrating violation to FCHC policies and procedures.
14. Addresses unanticipated clinic wide problems with appropriate teams.
IV. PROVIDER RESPONSIBILITIES:
1. Provides comprehensive primary medical care including examination, diagnosis, treatment, patient counseling, referral and medical follow-up, including lab result and diagnostic test interpretation.
2. Provides consultation to Nurse Practitioners, Nurse Midwives and Physician Assistants.
3. Provides telephone consultation to patients and families as necessary.
4. Maintains appropriate policies, protocols, records, reports, and statistical data relevant to services provided.
5. Contributes to the quality and maintenance of on-site clinical support services such as laboratory, pharmacy, emergency kit, specialty referrals; as well as for the arrangements pertaining to outside clinical support services such as x-ray, pharmacy, and specialty referrals.
6. Consults with staff members regarding the comprehensive health care needs of individual clients.
7. Participates in Health Center’s system of after-hours coverage including weeknight, weekend call and/or hospital in-patient rounds.
8. Plans and implements Health Center programs and projects in cooperation with identified staff members.
9. Participates in the development of in-service programs related to medical issues and other pertinent patient healthcare management issues.
10. Accepts assignments and responsibilities regarding time schedules for patient care, committee work, quality assurance and other areas related to special competence.
11. Obtains hospital admitting privileges for care of hospitalized patients or utilizes services hospitalist or residency in-patient services.
12. Precepts medical students, medical residents, nurse practitioner students and physician assistant students.
V. PERIODIC DUTIES:
1. Contributes to Health Center community health activities outside of regular job responsibilities.

4. Medicare and Medicaid provider enrollment numbers including Medicare UPIN number.
5. Board Eligible or Board-Certified status preferred.
6. Completion of residency training program in primary care specialty preferred.

III. PRIMARY RESPONSIBILITIES:
A. Participates as an integral part of a multi-disciplinary team for the comprehensive health care of clients and their families.
- Provides triage assessment, consultation, and referral for FCHC clients contacting FCHC through the telephone, patient portal and directly in clinic.
- Consults with medical provider as needed regarding client status to determine the need for care and/or level of care.
- Schedules same day appointments in conjunction with provider.
- Provides appropriate education and home care advice.
- Obtains information for clients requesting test results and medication refills.
- Documents assessment, patient education, nursing interventions, and encounter notes in the EMR.
- Sends refill requests to appropriate provider and completes prior authorization if needed by insurance.
- Performs routine direct patient care such as administering injections, dressing changes, vital sign assessment and other requested and necessary RN duties.
- Develops individualized care plan setting mutual health goals with patient/family for patients with acute conditions, complex conditions and co-occurring chronic illnesses.
- Provides education about disease processes and management (e.g. diabetes, asthma, and COPD). Emphasizing disease prevention and health promotion and addressing the specific needs of the client.
- Consults with members of multidisciplinary care team (Community Health, Nutrition, Behavioral Health, Pharmacy, etc.) about identified health conditions and the tailored interventions necessary to address barriers and challenges to achieving individual health care goals.
- Coordinates care using a care management approach, including risk assessment, coordination and referral, follow-up and tracking, crisis intervention, documentation and modification of care plan as needed, to maintain continuity of patient care.
- Provides medication, education and support to help patients better manage all of their health conditions.
- Performs post-hospital discharge transitions of care encounters and oversees the scheduling of patient for follow-up as appropriate and per clinical protocols (e.g. new onset of illness or diagnosis). Documents transition of care encounter, including medication reconciliation, in the EMR.

- Works with providers to support team efforts around care coordination and the model for improving metrics established by Health Resources & Services Administration (HRSA), Primary Care Health Home (PCHH), Accountable Care Organization (ACO), and others as identified in partnership or regulatory compliance.
- May attend learning sessions and collaborative calls with FCHC partners, such as Missouri Primary Care Association, Department of Health and Senior Services, and others, to facilitate best practices within the RN role.
- May interface with DRVS and Cyber Access to monitor effectiveness of individual care plans for patients with Medicaid coverage.
B. Provides essential support and assistance to other RN team members to ensure team success and consistent and quality patient care.

- Experience working with patients in a primary care setting. OB experience preferred.

Podiatrist
I. BASIC FUNCTION:
Responsible for providing comprehensive podiatric medical care to patients of Family Care Health Centers (FCHC), including evaluation, diagnosis, treatment, and prevention of foot and ankle conditions. The Podiatrist works collaboratively with medical providers, nursing staff, and interdisciplinary care teams to support high-quality, patient-centered care in alignment with FCHC’s mission and clinical standards.

JOB DESCRIPTION
DEPARTMENT: HEALTH OPERATIONS
JOB TITLE: LEAD MEDICAL ASSISTANT
LEAD MEDICAL ASSISTANT
III. PRIMARY RESPONSIBILITIES:
1. Works with the care team and performs duties such as pre-visit planning, administers Screening, Brief Intervention and Referral to Treatment (SBIRT), depression screening, tobacco screening and follow-up, BMI assessment and follow-up and immunizations, as well as assists with providing educational materials to patients and enters data into the electronic system
2. Records data into structured data fields and follows standard operation procedures for system documentation. Administers vaccines and documents all required data in the designated fields, according to insurance eligibility.
3. Coordinates patient flow with the front desk receptionist and the providers.
4. Performs procedures such as drawing blood and completing lab work, injections, EKG’s, vision and hearing screenings, ear irrigations, etc.
5. Records vital signs and other pertinent data on client records.
6. Prepares patients for general and specific examinations.
7. Maintains the inventory of all instruments and supplies, excluding medications. Responsible for the daily preparation of exam rooms.
8. Assists providers in treatments, diagnostic exams and minor surgical interventions.
9. Utilizes effective communication and listening skills. Demonstrates respect for diversity in approaching patients and families.
10. Maintains the organization of the clinical area environment.
11. Utilizes time effectively when not assigned to a provider who’s actively seeing patients.

Lead Medical Assistant for FCCC/FPSE:
1. Under the supervision of Clinical Nurse Manager responsible for the direct management of Medical Assistants. This includes the completion of annual Meical Assistant skills checklists.
2. Assists Clinical Nurse Manager with feedback for hiring, disciplinary action, terminations and conducting performance evaluations.
3. Responsible for maintaining the CLIA and OSHA lab standards, including testing requirements.
4. Responsible for maintaining safety checks and calibration of clinical equipment, ie. Sterilizer.
5. Responsible for interpreting/translation for FCHC patients, if applicable.
6. Responsible for onsite supervision for lab testing, supply ordering, vaccine ordering, calibration of clinical equipment, and day to day Medical Assistant operations in the clinical area.
7. This position will be an expert as a “super user” for the EPIC electronic system. Also, this position will be a trainer who will assist in the transformation of the Medical Assistant role.

Actively participates and leads initiatives in conjunction with other departments including health fairs, health promotion projects, and outreach events.
10. Attends nutrition department meetings, trainings and continuing education opportunities in order to stay up to date on evidence-based scientific guidelines for patient care.
11. Serves as preceptor for multiple dietetic internship programs.
IV. PERIODIC DUTIES:
1. Plans health and nutrition related events/activities as appropriate.
2. Provides assistance with scheduling appointments and WIC benefit issuance, as needed.
3. Provides education and mentoring opportunities to co-workers and other healthcare staff to support professional development and improve quality of patient care.
3. Performs other duties as assigned.
V. WORKING RELATIONSHIPS:
Inside Health Center: All inclusive.
Outside Health Center: Patients, health care professionals, and other community health agencies.
VI. QUALIFICATIONS:
1. Master’s degree in Nutrition or related field required.
2. Completion of a dietetic internship program accredited or approved by The Academy of Nutrition & Dietetics – Accreditation Council for Education in Nutrition and Dietetics (ACEND) resulting in a verification statement from an Internship Program in Nutrition and Dietetics.
3. Registered Dietitian or RD eligible required.
4. One to two years experience in a community health setting preferred.
VII. CONTINUING EDUCATION:
Continued education in nutrition-related field is required.
VIII. PROFESSIONAL MEMBERSHIPS:
Academy of Nutrition & Dietetics, Missouri Academy of Nutrition & Dietetics, Academy of Nutrition & Dietetics St. Louis, encouraged.
