Medical

Certified Medical Assistant

Family Care Health Centers is looking for FULL-TIME (8:00 AM to 5:00 PM) and PART-TIME (12:00 PM to 6:00 PM) Medical Assistants to work as an integral role in our Health Operations department. At Family Care Health Centers we work to provide affordable and accessible comprehensive primary care services to anyone, with emphasis on the medically underserved, and to train a primary care workforce to promote the general health of our community members.

Our hours of operation are Monday through Friday from 8:00 to 5:00. Medical Assistants will work a 40 hour work week with 8 hour shifts per day.

Operations Specialist - Front Desk,Scheduler, Medical Records

Our Health Operations team is looking for top talent in Metro St. Louis to work as an Operations Specialist! As a member of the Operations team, you have the opportunity to learn different aspects of the health care field. This includes being cross trained to work as a(n): Operator, Front Desk Receptionist, Appointment Scheduler, Correspondence Clerk, or Medical Records Clerk. Here at FCHC it is our mission to provide affordable and accessible comprehensive primary care services to anyone, with an emphasis on the medically underserved, and to train a primary care workforce in order to promote the general health of the service area.

Responsibilities:

  • Handling all telephone system changes, ie. Telephone numbers, changing messages, as well as acting a system troubleshooter.
  • Receiving and answering all incoming phone calls, scheduling patient appointments, and creating tasks with detailed messages for all staff.
  • Screening calls for providers and routing them to the appropriate clinical staff.
  • Greeting incoming patients and visitors utilizing excellent customer service skills
  • Ensures that consent forms are completed by patients annually.
  • Obtains financial documentation from each patient/family unit to acquire information for the Center’s Sliding Fee Scale Program.
  • Utilizes the telephone and paging system to help patients receive services they need.
  • Creates and updates patient Electronic Health Records (EHR).
  • Assist patients in participating in special programs by giving out applications and assisting patients in the completion of applications.
  • Payment collections
  • Schedules appointments for patients in person or via telephone, monitors cancellations, and reschedules appointments.
  • Log all incoming and outgoing requests for information about the patient’s HER.
  • Copy patient charts as requested and notify Custodian of Records immediately when patient charts are requested from an outside source.

Dental

Dentist

As a Federally Qualified Health Center, FCHC Dentists are eligible for the National Health Services Corps loan repayment program. This program repays up to $50K for just two years of service!

As a member of FCHC, the Dentist is responsible for overseeing the development, management, and provision of primary oral health services and education. It is also the responsibility of the Dentist to provide comprehensive dental care including examination, diagnosis, treatment, patient counseling, referral, and follow-up.

Dental Assistant

Dental Assistants are vital to our Dental team as they maintain the overall physical operations of the clinical area and assist the Dentist in the delivery of dental care services. The Dental Assistant, in collaboration with the dental care team, is accountable for assisting our patients towards their highest level of health. Additional responsibilities include:

  • Coordinating patient flow with the receptionist and dental providers.
  • Assisting the Dental Receptionist on an as needed basis.
  • Perform procedures such as x-ray exposure and development, fluoride therapy, patient study and working models, etc.
  • Maintains the inventory and proper sterilization of all instruments and supplies, excluding medicines, and prepares rooms daily.
  • Assist dental providers in treatments, diagnostic exams, and surgical interventions.
  • Uses four-handed dental technique.
  • Taking vital signs- blood pressure, temperature, pulse, and respiration taking skills.
  • Knowledge and proper handling and mixing of dental materials, cements, etc.

Behavioral Health

Manager of Behavioral Health Consultation Services (Pediatrics), PhD

Are you someone who prides themselves on their leadership skills?

Are you looking to not only lead a team, but practice pediatric psychology at the same time?

If so, Family Care Health Centers (FCHC) is looking to add a Manager of Behavioral Health Consultation Services to lead the members of our Behavioral Health Consultation team. Responsibilities include, but are not limited to:

  • To collaborate with the primary care providers, pediatric care managers and other members of the team (nurses, MAs, CHWs) in the delivery of comprehensive and integrated behavioral health care services.
  • To provide same-day behavioral health consultation for patients at FCHC, as requested by their primary care provider (PCP), with an emphasis on pediatric patients needing behavioral assessment, diagnostic evaluation or brief interventions.
  • To provide more comprehensive assessments (including psychological testing) for pediatric patients whose needs cannot be met by same-day consultations alone.
  • To meet the target number of encounters per year agreed upon with supervisor and to attach the appropriate billing code to each encounter.
  • To lead the Behavioral Health Consultants (BHC)
  • To support the CBHO in the system-wide integration of BH into primary care, including ongoing QI related to processes and encounter targets, billing procedures, management of grant funded projects, and planning for monthly BH meetings and the annual BH department retreat.

Qualifications:

  • Ph.D. in clinical psychology – REQUIRED.
  • MO state licensure – REQUIRED.
  • Experience in primary care integration during graduate school, internship, fellowship and/or professional practice – REQUIRED.

Behavioral Health Care Manager (Adult)

Do you pride yourself on your ability to assist patients with navigating through their behaviors, life goals, and their health goals?

Are you interested in working with a diverse group of patients?

If so, Family Care Health Centers is looking to add two (2) Behavioral Health Care Manager (Adult) to our Behavioral Health team. As a Behavioral Health Care Manager (Adult) you are responsible for providing care management for patients of FCHC and for clinical supervision of non-licensed care managers. Additional responsibilities include but are not limited to:

  • Function as the BH Care Manager within the integrated primary care and behavioral health team.
  • In coordination with the BHC and Primary Care Provider (PCP), enroll eligible and interested patients into the Circle of Care project.
  • Once the patient is enrolled, track patient progress towards grant outcomes.
  • As often as clinically appropriate, face-to-face visits and telephone calls with Circle of Care patients and other members of the patient’s network over the course of their enrollment in the project (3 to 12 months).
  • Establish and maintain patient registry to monitor clinical outcomes as well as key processes of care.
  • Arrange for inpatient, partial hospitalization, intensive outpatient programs as needed.
  • Attend care team meetings and document clinical information/decision-making in the patient registry.
  • Facilitate referrals to other outpatient therapists as indicated.
  • Facilitate referrals to psychiatry as needed.
  • Bill Care management services when appropriate.

Qualifications:

  • Master’s Degree in Social Work – REQUIRED.
  • LCSW (Licensed Clinical Social Worker) – REQUIRED.
  • Minimum of two (2) years experience working in the field of social work or behavioral health care is ~ REQUIRED.
  • Strong interpersonal and communication skills are ~ REQUIRED.

Community Dietitian

Community Dietitians are vital to our Community Health Center as they provide Medical Nutrition Therapy services to patients of all ages.

Core Responsibilities:

· Provide participant-centered nutrition assessment, education, and counseling for health center patients and individuals enrolled in the WIC program.

· Assist in the development and implementation of WIC Local Agency Plan to maintain WIC caseload and improve health outcomes of WIC population.

· Provide support to promote and protect breastfeeding as an optimal infant feeding practice.

WIC Certifier

Family Care Health Centers is seeking a full-time experienced WIC Certifier to work as an integral role in our growing health care team environment. These positions provide clerical support (answering telephone, scheduling appointments, etc.) for the Women, Infants, and Children (WIC) Supplemental Nutrition Program. This position performs enrollment assessments and issues benefits as defined by Missouri Department of Health and Senior Services, Bureau of WIC and Nutrition Services.

This includes, but is not limited to:

  • collecting and reviewing documents to determine program eligibility
  • collecting and recording heights, weights, and finger sticks and assessing immunization records
  • navigate multiple computer systems, including electronic health record, and keep detailed records
  • communicate effectively with other members of the team to coordinate patient flow and create an efficient system and quality program
  • refer clients to appropriate community services such as food pantries, SNAP, Medicaid, etc.
  • promote and support a woman’s decision to breastfeed by maintaining a positive attitude and making appropriate referrals
  • attend staff meetings and on-going training opportunities as offered
  • work at both sites and must be able to work evenings and Saturdays when necessary.

THe position is contingent upon continued WIC Funding. COVID vaccine, FLU vaccine and PPD screening are required for employment at FCHC. The position reports to the Nutrition Services Director.

Community Health

Community Health Worker

Family Care Health Centers is seeking a full-time experienced Community Health Worker.  This position provides community-based support services for clients and their families in accordance with Center policies and protocols utilizing the corporate compliance plan as a guide.

  • Makes home visits to clients based on referrals from providers, nurse educators and behavioral health team members.
  • Support services aim to assist clients with chronic conditions adopt healthy behaviors.
  • Also, acts as an advocate for the rights of culturally diverse clients and their families in the health care system and community.

Maintenance Technician

  • Providing general oversign and monitoring of janitorial, pest control, and climate control contractors; recommends changes to Chief Operations Officer/ Operations Direct.
  • Provides general in-house painting and/or supervision of contracted painters.
  • Provides in house carpentry, clinical and office furniture moving, basic electric and plumbing needs; recommends need for outside contractors for aforementioned services.
  • Arranges conference rooms and waiting areas for scheduled meetings.
  • Oversees parking lot maintenance with general weed control and gardening, recommending additional maintenance needs to the team leaders.
  • Transport office equipment between sites.
  • Responds to report of security monitoring system after hours alarm notification and contracts Operations Director or Administrator on Call for guidance regarding response.
  • Operates and oversees maintenance of FCHC Maintenance Vehicle; insures tags and licenses are current.
