Help keep our patients, staff and yourself safe by limiting contact at the office. Save time before your appointment by checking-in through your phone, iPad, or computer from your home or car! You can check in through a registration link sent to you by text/email from Phreesia, up to 3 days ahead of your appointment.

What is Phreesia for?

With the Phreesia, you can complete COVID-19 screening, complete intake forms, and make co-pay and balance statements. Patients will also receive a text about intake instructions to avoid the waiting room as much as possible.

How will Phreesia work?

Once you’re scheduled for an appointment, Phreesia will automatically send you a text or email that invites you to pre-register. Click the link and follow the instructions! It’s easy! Our forms will be available in English, Spanish, and Vietnamese.

What if I don’t have a smartphone/computer?

Don’t worry! Come to the office early before your appointment and see the front desk staff for your paperwork.

When will I receive a text message/email to complete my registration?

Phreesia will send out messages 3 days in advance of your appointment, with a reminder message one day in advance.
