Registered Nurse Referral Manager

Thank you for your interest in joining Family Care Health Centers.  Our patient-centric care team is looking for an experienced Registered Nurse to supervise our Referral Specialist team.  The ideal candidate has experience as a Referral Specialist with supervisory/management experience and has exceptional customer service, relationship management, and communication skills.  The position is located at our Carondelet center at 401 Holly Hills Avenue, St. Louis, MO 63111.

JOB DESCRIPTION:

The RN Referral Manager is responsible for supervising the Referral Specialist team and providing clinical oversite for the referral process.  In addition, the RN Referral Manager oversees the hiring, onboarding, training, and evaluation of the Referral Specialist team members.  Referrals Management also involves making referrals to hospitals and other outside agencies in addition to working with internal department managers related to internal referrals.

RESPONSIBILITIES INCLUDE:

  • Overseeing the entire referral process and responding to issues with an action plan.
  • Ensuring timely responses to all hospitals and other providers of care as directed by FCHC providers.
  • Ensures imaging results are tracked and follow-up is completed on overdue results.
  • Oversees referral team, including follow-up on untimely responses or if other problems occur.  Responds with an action plan to address issues.
  • Collaborates with health care providers at outside entities ensuring that the system of processing referrals is working wll, minimizing risk to patients and providers.
  • Maintains up-to-date working knowledge of routines and requirement of health care providers with whom referrals are coordinated.
  • Communicates updates and changes related to the referral process or provider requirements to the associate medical director, CMO, and other members of the multidisciplinary care team as appropriate.

QUALIFICATIONS INCLUDE:

  • Graduate of an accredited school of nursing is required.
  • Licensure as a Registered Nurse in the State of Missouri, in good standing, is required.
  • Excellent verbal and written communication skills required.
  • 3+ years of work experience in a primary health care setting preferred.
  • 2+ years of supervisory or management experience preferred.
  • Ability to commute to our Carondelet center at 401 Holly Hills Avenue, St. Louis, MO 63111.

SALARY:

  •  From $33.78, depending on experience.
