Medical

Certified Medical Assistant

Family Care Health Centers is looking for FULL-TIME (8:00 AM to 5:00 PM) and PART-TIME (12:00 PM to 6:00 PM) Medical Assistants to work as an integral role in our Health Operations department. At Family Care Health Centers we work to provide affordable and accessible comprehensive primary care services to anyone, with emphasis on the medically underserved, and to train a primary care workforce to promote the general health of our community members.

Registered Nurse (RN) Women's Health (OB/GYN)

Patient Care Coordinator

We’re seeking a Patient Care Coordinator (PCC) to join our team and play a key role in ensuring our patients receive the highest quality care. As a vital member of the Health Center’s care team, you’ll help improve clinical outcomes through seamless coordination and communication—working with providers, patients, and specialists to ensure everything runs smoothly!

What You’ll Do:

As a Patient Care Coordinator, your role will be dynamic and impactful, combining patient outreach, chart reviews, and care coordination to ensure that all our patients are getting the care they need. Your responsibilities include (comprehensive list):

  • Patient Outreach:
    • Contact patients via reminder letters, phone calls, or the patient portal to schedule appointments and remind them about overdue care (procedures, lab tests, and appointments).
    • Perform chart reviews to make sure that all clinical quality measures (CQM) and relevant information are up to date before reaching out to patients.
    • Document outreach efforts and update patient information in the EHR (Electronic Health Record) and other systems to ensure teams are informed about upcoming or overdue care.
  • Chart Review, Care Coordination & Follow-Up:
    • Perform EHR chart reviews for quality measures, reconciling with Excel spreadsheets as required for ACO and other clinical quality reporting.
    • Gather and prepare relevant notes from specialists and hospital visits to ensure patients are ready for upcoming appointments.
    • Coordinate with providers to ensure patients are meeting quality metrics and follow up as needed to complete care tasks.
  • Assist the Nursing Team:
    • Answer patient phone calls, schedule appointments, and provide assistance as directed by the nursing team.
    • Help prepare new patients for their first visit with the FCHC PCP by obtaining their medical records and ensuring their charts are ready for the visit.
    • Check Cyber Access for medication adherence and hospitalizations to keep patient records up to date.

Our Health Operations team is looking for top talent in Metro St. Louis to work as an Operations Specialist! As a member of the Operations team, you have the opportunity to learn different aspects of the health care field. This includes being cross trained to work as a(n): Operator, Front Desk Receptionist, Appointment Scheduler, Correspondence Clerk, or Medical Records Clerk. Responsibilities:

  • Handling all telephone system changes, ie. Telephone numbers, changing messages, as well as acting a system troubleshooter.
  • Receiving and answering all incoming phone calls, scheduling patient appointments, and creating tasks with detailed messages for all staff.
  • Screening calls for providers and routing them to the appropriate clinical staff.
  • Greeting incoming patients and visitors utilizing excellent customer service skills
  • Ensures that consent forms are completed by patients annually.
  • Obtains financial documentation from each patient/family unit to acquire information for the Center’s Sliding Fee Scale Program.
  • Utilizes the telephone and paging system to help patients receive services they need.
  • Creates and updates patient Electronic Health Records (EHR).
  • Assist patients in participating in special programs by giving out applications and assisting patients in the completion of applications.
  • Payment collections
  • Schedules appointments for patients in person or via telephone, monitors cancellations, and reschedules appointments.
  • Log all incoming and outgoing requests for information about the patient’s HER.
  • Copy patient charts as requested and notify Custodian of Records immediately when patient charts are requested from an outside source.

Dentist

As a Federally Qualified Health Center, FCHC Dentists are eligible for the National Health Services Corps loan repayment program. This program repays up to $50K for just two years of service!

As a member of FCHC, the Dentist is responsible for overseeing the development, management, and provision of primary oral health services and education. It is also the responsibility of the Dentist to provide comprehensive dental care including examination, diagnosis, treatment, patient counseling, referral, and follow-up.

Dental Assistant

Dental Assistants are vital to our Dental team as they maintain the overall physical operations of the clinical area and assist the Dentist in the delivery of dental care services. The Dental Assistant, in collaboration with the dental care team, is accountable for assisting our patients towards their highest level of health. Additional responsibilities include:

  • Coordinating patient flow with the receptionist and dental providers.
  • Assisting the Dental Receptionist on an as needed basis.
  • Perform procedures such as x-ray exposure and development, fluoride therapy, patient study and working models, etc.
  • Maintains the inventory and proper sterilization of all instruments and supplies, excluding medicines, and prepares rooms daily.
  • Assist dental providers in treatments, diagnostic exams, and surgical interventions.
  • Uses four-handed dental technique.
  • Taking vital signs- blood pressure, temperature, pulse, and respiration taking skills.
  • Knowledge and proper handling and mixing of dental materials, cements, etc.

Behavioral Health Consultant - Psychologist (PEDIATRICS)

Are you passionate about providing behavioral health care to the underserved?

If so, Family Care Health Centers (FCHC) is adding a Psychologist to our Behavioral Health Services team!

Psychologists are responsible for providing same-day behavioral health diagnostic assessments and interventions for patients of FCHC. In addition, Psychologist are responsible for the following:

  • Collaborating with primary care providers, pediatric care managers, and other members of the team (nurses, MAs, and CHWs) in the delivery of comprehensive and integrated behavioral health care services.
  • Provide same-day behavioral health consultations for patients of FCHC as requested by their PCP.
  • Define problems accurately and use best practice to implement intervention rapidly.
  • Conduct assessments with a solid understanding of DSM -5-TR, family systems, the connection between physical and mental health, and the impact of trauma on patients and families.
  • Tailor PCP recommendations to the primary care setting; focus on recommendations that support the provider’s treatment of BH in primary care.
  • Employ knowledge of BH medications to help PCPs treat complex BH conditions and substance use disorders, including consultation with psychiatry (internally. MOCPAP).
  • To provide more comprehensive assessment (including psychological testing) for patients whose needs cannot be met by same-day consultations alone. To provide consultation to LCSWs, LMFTs, LPCs in the areas of diagnostic assessment, treatment, and primary care consultation skills in more complex cases.

Qualifications

  • Ph.D. in clinical psychology is required (from an APA accredited program).
  • Active MO State License – REQUIRED.
  • Minimum of 2 years experience working in the field.
  • Work experience or strong desire to work in urban community health setting preferred.

As a BHC – LMSW, the employee is responsible for providing same day behavioral health diagnostic assessment and interventions for patients of FCHC. Responsibilities for this role include, but are not limited to:

  • Collaborating with Primary Care Providers (PCPs), BH Care Managers, and other members of the team (RNs, MAs, CHWs) in the delivery of comprehensive and integrated behavioral health care services.
  • Providing same-day behavioral health consultation for patients of FCHC as requested by the PCP, including assessment, diagnostic evaluation, and brief intervention.
  • Provides presentations to PCPs on BH topics of interest.
  • Will supervise Graduate Students in Social Work or LMSWs, if applicable.

Clinical Practice Skills:

  • Conduct assessments with a solid understanding of DSM-5-TR, family systems, the connection between physical and mental health, and the impact of trauma on patients’ families.

Behavioral Health Care Manager (Adult)

If so, Family Care Health Centers is looking to add two (2) Behavioral Health Care Manager (Adult) to our Behavioral Health team. As a Behavioral Health Care Manager (Adult) you are responsible for providing care management for patients of FCHC and for clinical supervision of non-licensed care managers. Additional responsibilities include but are not limited to:

  • Function as the BH Care Manager within the integrated primary care and behavioral health team.
  • In coordination with the BHC and Primary Care Provider (PCP), enroll eligible and interested patients into the Circle of Care project.
  • Once the patient is enrolled, track patient progress towards grant outcomes.
  • As often as clinically appropriate, face-to-face visits and telephone calls with Circle of Care patients and other members of the patient’s network over the course of their enrollment in the project (3 to 12 months).
  • Establish and maintain patient registry to monitor clinical outcomes as well as key processes of care.
  • Arrange for inpatient, partial hospitalization, intensive outpatient programs as needed.
  • Attend care team meetings and document clinical information/decision-making in the patient registry.
  • Facilitate referrals to other outpatient therapists as indicated.
  • Facilitate referrals to psychiatry as needed.
  • Bill Care management services when appropriate.

Qualifications:

  • Master’s Degree in Social Work – REQUIRED.
  • LCSW (Licensed Clinical Social Worker) – REQUIRED.
  • Minimum of two (2) years experience working in the field of social work or behavioral health care is ~ REQUIRED.

Pharmacy technicians are also responsible for data entry, insurance adjudication, and inter-office communications. The pharmacy technician, in collaboration with the Chief Pharmacist, is accountable for assisting our patients towards their highest level of health. This is a full-time position within the Family Care Health Centers organization.

Pharmacy Technician Requirements:
