FCHC’s behavioarl health services are person-centered, trauma-informed and integrated with primary care. The behavioral health team provides care to children and adults, psychiatric services, substance use disorder support, health coaching, and case management to pediatric and adult patients. These services are accessible to FCHC patients through referrals by their primary care provider.

FCHC is proud to offer our patients the convenience of getting their prescriptions on-site typically within 15-minutes of their appointment. We also offer FREE home prescription delivery services to ALL of our patients.

As a federally qualified health center, we have the ability to provide prescriptions at lower costs than other pharmacies.

 

 

 

FCHC’s Nutrition Services department provides three key services: medical nutrition therapy, lactation services and the Missouri WIC program (Women, Infants & Children).

  • Registered Dietians provide personalized nutrition counseling to patients, at no cost
  • Dietitians focus the conversation and patient plan on creating measurable steps to achieve health goals specific to the patient
  • Clinical breastfeeding guidance is provided  by certified lactation consultants
  • Prenatal breastfeeding classes are offered  free of charge to patients
  • Program for nutrition and breastfeeding education and counseling, health screenings, community referrals, and healthy supplemental foods for prefnant, breastfeeding and postpartum women and infants/children up to age 5 years old

 

 

Our community health team is a comprised frontline of public health workers who are trusted members of and/or have an unusually close understanding of the community they serve. This trusting relationship enables community health workers to serve as lisaisons/links/intermediaries between health/social services and the community, to facilitate access to services and improve the quality and cultural competence of service delivery.

Supervises: Shares supervision of providers at a specific site with Chief Medical Officer. Medical consultation to Nurse Practitioners and Physician Assistants of same specialty, ie. Pediatrician to PNP, Family Physician to PNP and OB/GYN Practitioners.

DEPARTMENT: MEDICAL SERVICES

JOB TITLE: ASSOCIATE MEDICAL DIRECTOR/PRIMARY CARE

PHYSICIAN: INTERAL MEDICINE, FAMILY PRACTICE, OB/GYN, PODIATRY, PEDIATRICIAN, AND GENERAL PRACTICE ASSOCIATE MEDICAL DIRECTOR/PRIMARY CARE PHYSICIAN

III. PRIMARY RESPONSIBILITIES:

1. Recruits, interviews, and recommends hiring of providers employed at a respective site to Chief Medical Officer.
2. Reviews and coordinates Continuing Medical Education (CME), annual leave, personal leave and unpaid leave. Makes recommendations to Chief Executive Officer for leave requiring Chief Executive Officer approval.
3. Coordinates clinical scheduling; shares development of on-call schedule and hospital rounding with Chief Medical Officer.
4. Directs disciplinary action, conflict resolution, and incident reporting involving providers. All incident reports must go to the Chief Medical Officer for signature prior to being forwarded to the Chief Executive Officer and CQI Committee.
5. Monitors and remedies provider productivity and patient satisfaction issues.
6. Provides orientation for new providers in conjunction with Chief Medical Officer and Human Resource Director.
7. Makes recommendations to Chief Medical Officer and Chief Executive Officer regarding provider contract renewals and terms of renewals, as well as performs performance reviews of team.
8. Ensures agency productivity, financial and patient satisfaction goals are attained.
9. Coordinates, facilitates and/or attends Department Head/Team Leader, provider, and site meetings..
10. Attends Board of Directors meetings at invitation of Chief Executive Officer.
11. Monitors providers’ clinical staffing to ensure adequate access for all patients during all hours of operation; works in a tandem with Operations Supervisor(s) and Chief Operations Officer to ensure unified agency effort in providing adequate access to services.
12. Serves as conduit between providers and Human Resource Director to ensure provider benefits are managed according to FCHC Personnel Policies.
13. Supports and/or participates in the discharge committee process of patients demonstrating violation to FCHC policies and procedures.
14. Addresses unanticipated clinic wide problems with appropriate teams.

 

IV. PROVIDER RESPONSIBILITIES:

1. Provides comprehensive primary medical care including examination, diagnosis, treatment, patient counseling, referral and medical follow-up, including lab result and diagnostic test interpretation.
2. Provides consultation to Nurse Practitioners, Nurse Midwives and Physician Assistants.
3. Provides telephone consultation to patients and families as necessary.
4. Maintains appropriate policies, protocols, records, reports, and statistical data relevant to services provided.
5. Contributes to the quality and maintenance of on-site clinical support services such as laboratory, pharmacy, emergency kit, specialty referrals; as well as for the arrangements pertaining to outside clinical support services such as x-ray, pharmacy, and specialty referrals.
6. Consults with staff members regarding the comprehensive health care needs of individual clients.
7. Participates in Health Center’s system of after-hours coverage including weeknight, weekend call and/or hospital in-patient rounds.
8. Plans and implements Health Center programs and projects in cooperation with identified staff members.
9. Participates in the development of in-service programs related to medical issues and other pertinent patient healthcare management issues.
10. Accepts assignments and responsibilities regarding time schedules for patient care, committee work, quality assurance and other areas related to special competence.
11. Obtains hospital admitting privileges for care of hospitalized patients or utilizes services hospitalist or residency in-patient services.
12. Precepts medical students, medical residents, nurse practitioner students and physician assistant students.

 

V. PERIODIC DUTIES:

1. Contributes to Health Center community health activities outside of regular job responsibilities.
2. Participates in Health Center staff problem solving groups.
3. Attends and participates in department meetings, etc.
4. Performs other duties as assigned.

 

VI. WORKING RELATIONSHIPS:

Inside Health Center: All inclusive; collaboration with all direct reports.

VII. QUALIFICATIONS:

1. Doctor of Medicine or Doctor of Osteopathy required.
2. License to practice medicine in the State of Missouri and be in good standing required.
3. Drug licensed including DEA and BNDD numbers required.
4. Medicare and Medicaid provider enrollment numbers including Medicare UPIN number.
5. Board Eligible or Board-Certified status preferred.
6. Completion of residency training program in primary care specialty preferred.
7. Work experience or strong desire to work in urban community health setting preferred.

JOB DESCRIPTION

DEPARTMENT: HEALTH OPERATIONS

JOB TITLE: REGISTERED NURSE ~ RN

REGISTERED NURSE – RN

 

III. PRIMARY RESPONSIBILITIES

A. Participates as an integral part of a multi-disciplinary team for the comprehensive health care of clients and their families.

  1. Provides triage assessment, consultation, and referral for FCHC clients contacting FCHC through the telephone, patient portal and directly in clinic. 
  2. Consults with medical provider as needed regarding client status to determine the need for care and/or level of care. 
  3. Schedules same day appointments in conjunction with provider. 
  4. Provides appropriate education and home care advice. 
  5. Obtains information for clients requesting test results and medication refills. 
  6. Documents assessment, patient education, nursing interventions, and encounter notes in the EMR. 
  7. Sends refill requests to appropriate provider and completes prior authorization if needed by insurance.
  8. Performs routine direct patient care such as administering injections, dressing changes, vital sign assessment and other requested and necessary RN duties.
  9. Develops individualized care plan setting mutual health goals with patient/family for patients with acute conditions, complex conditions and co-occurring chronic illnesses.
  10. Provides education about disease processes and management (e.g. diabetes, asthma, and COPD). Emphasizing disease prevention and health promotion and addressing the specific needs of the client. 
  11. Consults with members of multidisciplinary care team (Community Health, Nutrition, Behavioral Health, Pharmacy, etc.) about identified health conditions and the tailored interventions necessary to address barriers and challenges to achieving individual health care goals. 
  12. Coordinates care using a care management approach, including risk assessment, coordination and referral, follow-up and tracking, crisis intervention, documentation and modification of care plan as needed, to maintain continuity of patient care. 
  13. Provides medication, education and support to help patients better manage all of their health conditions.
  14. Performs post-hospital discharge transitions of care encounters and oversees the scheduling of patient for follow-up as appropriate and per clinical protocols (e.g. new onset of illness or diagnosis). Documents transition of care encounter, including medication reconciliation, in the EMR.

  12. May attend learning sessions and collaborative calls with FCHC partners, such as Missouri Primary Care Association, Department of Health and Senior Services, and others, to facilitate best practices within the RN role.
  13. May interface with DRVS and Cyber Access to monitor effectiveness of individual care plans for patients with Medicaid coverage.

B. Provides essential support and assistance to other RN team members to ensure team success and consistent and quality patient care.

C. As part of the health care team, responds to medical emergencies in the office.

D. Acts as a resource for Medical Assistants and other personnel as needed.

E. Participates in facilitating a learning environment for students.

F. Collaborates with self-directed management teams to continuously improve the delivery of health services.

  4. Experience working with patients in a primary care setting. OB experience preferred.

        Manager of Clinical Support - RN

        I. BASIC FUNCTION:

        The Manager of Clinical Support (MCS) leads a team of Registered Nurses (RN), Patient Care Coordinators (PCC) and Medical Assistants (MA). This position is responsible for ensuring that the clinical team is providing excellent and comprehensive outpatient health care services to FCHC patients and their families. The MCS will report to the Director of Nursing (DON) and collaborate with providers, Health Center Administrators (HCA), operations and other department managers to deliver premier care to the community and patients we serve.

         

        II. INTERDEPARTMENTAL RELATIONSHIPS:

        • Department Officer: Chief Operations Officer
        • Reports to: Director of Nursing
        • Direct Reports: Registered Nurses, Patient Care Coordinators, and Medical Assistants

         

        III. PRIMARY RESPONSIBILITIES:

        • This individual is responsible for educating, training and supporting the clinical team using population health strategies.

        While focused on the daily tasks and flow of the clinic, this role will identify process improvement activities, create workflows, implement change and monitor progress to maximize patient care. The ability to quickly adapt and address clinical needs of the patient and support staff are essential. The MCS works closely with the DON to support the delivery of safe, high-quality patient care across all nursing services. In addition, this individual will collaborate with the DON to align clinical support operations with nursing leadership priorities, organizational goals and regulatory requirements.

        1. Greets incoming patients and visitors utilizing excellent customer service skills as noted under Service Standards.

        2. Ensures all necessary consent forms are completed by patients annually.

        3. Obtains financial documentation from each patient/family unit in order to acquire information necessary for the Center’s Sliding Fee Scale Program. Obtains insurance information and verifies coverage prior to patient appointments.

        4. Utilizes the telephone and paging system to help patients receive services as necessary.

        5. Creates and updates patients’ electronic health record (EHR). This includes scanning the registration forms which are reviewed for completion and placed into the appropriate categories in the electronic health record.

        6. Updates patient record for changes in any demographic information. Makes appropriate changes in the electronic health records (EHR).

        7. Connects patients to the appropriate resource to handle or answers patients’ inquiries regarding sliding fee schedule, insurance, collections, etc.

        8. Assists patients in participating in special programs by giving out applications and assisting patients in the completion of applications.

        9. Collects payments of fees for current or previous visits. Provides receipts of payments to patients.

        10. Promotes activities such as patient portal enrollment to improve clinic engagement.

        11. Submits money collected and payment journal at the end of the day.

        12. Secures money drawers at all times.

        13. Performs other duties as assigned.

         

        C) APPOINTMENT SCHEDULER:

         

        1. Schedules appointments for patients in person or via telephone, monitors cancellations, and reschedules appointments, primarily responsible for the recall list, if applicable.

        2. Schedules appointments for specific patient populations at the direction of clinical staff.

        3. Performs other duties as assigned.

         

        D) MEDICAL RECORDS CLERK:

         

        1. Ensures all loose chart forms, including consult notes, progress notes, lab reports, and immunizations are scanned and filed appropriately on the electronic health records. These documents are imported into the patient’s electronic health record and forwarded to staff at FCHC following FCHC’s medical standard operating procedure (SOP) for FCHC scanning procedure.

        2. Requests medical records from outside entities as directed by the provider.

        3. Downloads medical records from health record systems and imports medical records into a patient’s electronic health record. These records are downloaded, imported and forwarded to staff FCHC following FCHC’s medical records standard operating procedure.

        4. Faxes appropriate correspondence as needed.

        5. Picks up medical records information from providers and nursing staff and faxes, mails or delivers electronically any necessary information to outside entities as requested by provider.

        6. Performs other duties as assigned.

         

        F) INTERPRETER SERVICES:

         

        1. Schedules interpreters for all patients (both in person and utilizing Video Remote Interpreting – VRI).

        2. Schedules transportation for all patients as needed.

        3. Verifies insurance for all non-English speaking patients. Works with non-English speaking patients on identifying health insurance options.

        4. Performs other duties as assigned.

         

        IV. LEAD FRONT DESK ASSOCIATE RESPONSIBILITIES:

        1. Prepares work schedules for Operations Staff, ensuring appropriate staff levels.

        2. Provides direction, instruction, and support to Operations Staff during Operations Director’s and/or Patient Access Manager’s absence.

        3. Provides orientation, training, implementation, and coordination of staff. Orients staff to changes in work procedures and duties.

        4. Maintains medical records and ensures the availability of medical records

        5. Oversees the registration processes.

        6. Functions as fill-in for all positions performed by Operations Staff as needed.

        7. Helps with general site operations including building maintenance and security, equipment and supply maintenance and basic financial functions, time clock and payroll, etc., as required.

        8. Provides feedback to Operations Director and/or Patient Access Manager for staff performance evaluations.

        9. Handles patient grievances and assists Operations Director and/or Patient Access Manager with staff issues.

         

        V. PERIODIC DUTIES:

        1. Contributes to Health Center community health activities outside of regular job responsibilities.

        2. Participates in Health Center staff problem solving groups.

        2. Attends and participates in department meetings, etc. as assigned.

        3. Performs other duties as assigned.

         

        VI. WORKING RELATIONSHIPS:

        Inside Health Center: All inclusive.

        Outside Health Center: Patients/Clients, students, vendors, community health agencies, hospitals, insurance companies, Medicare/Medicaid, physician offices, etc.

         

         

        VII. QUALIFICATIONS:

        1. High School Diploma or G.E.D. equivalent required.

        2. One (1) year medical office work experience or equivalent required.

        3. Five (five) years medical office experience preferred.

        4. Ability to type 40 WPM preferred.

        5. Knowledge of medical terminology preferred.

        6. Must possess excellent computer, communication and customer service skills.

        7. Must possess excellent organizational skills.

        8. Bi-lingual speaking skills a plus.
