If you had an appointment scheduled that was supposed to be in-person, a member of our team will call to notify you that it will now be a telehealth visit. If you have not received a phone call, or if you are unsure, please call our office to clarify.  The Carondelet Center on Holly Hills can be reached at 314-353-5190.  The Forest Park Southeast Center on Manchester can be reached at 314-531-5444.

1-2 days before your appointment, a member of our team will reach out to you to gather some information from you (your current phone number, address, insurance, etc.). During that phone call, our team member will remind you that appointment will take place via telehealth. Please make sure to give an accurate phone number or email address. This is how your provider will contact you for your appointment.

You will receive either a text/email from your provider to join the virtual visit on an application called MyChart OR you will receive a phone call from your provider.

Make sure you have reasonable privacy for the duration of your appointment. Please plan to be at home. Avoid public places or joining the visit while driving.
