Medical

Certified Medical Assistant

Family Care Health Centers is looking for FULL-TIME (8:00 AM to 5:00 PM) and PART-TIME (12:00 PM to 6:00 PM) Medical Assistants to work as an integral role in our Health Operations department. At Family Care Health Centers we work to provide affordable and accessible comprehensive primary care services to anyone, with emphasis on the medically underserved, and to train a primary care workforce to promote the general health of our community members.

Our hours of operation are Monday through Friday from 8:00 to 5:00. Medical Assistants will work a 40 hour work week with 8 hour shifts per day.

Patient Access Manager

FCHC is looking for a full-time Patient Access Manager!

The Patient Access Manager manages medical records in addition to supporting the Patient Access Team, which includes: Registration and Centralized Scheduling. The Patient Access Managers are responsible for the management of daily activities within the Operations Department which includes Call Center management, patient appointment scheduling, appointment reminder system, interpretation services, telephone system, remote interpreter services, medical records and correspondence. The ideal candidate must be enthusiastic, motivated, hard-working and flexible, be able to multi-task, have a great attitude, thrive in a fast-paced work environment and must be a rapid learner. The Patient Access Manager, in collaboration with the health care team, is accountable for assisting our patients toward their highest level of health. Dental

Dentist

As a Federally Qualified Health Center, FCHC Dentists are eligible for the National Health Services Corps loan repayment program. This program repays up to $50K for just two years of service!

As a member of FCHC, the Dentist is responsible for overseeing the development, management, and provision of primary oral health services and education. It is also the responsibility of the Dentist to provide comprehensive dental care including examination, diagnosis, treatment, patient counseling, referral, and follow-up.

Dental Assistant

Dental Assistants are vital to our Dental team as they maintain the overall physical operations of the clinical area and assist the Dentist in the delivery of dental care services. The Dental Assistant, in collaboration with the dental care team, is accountable for assisting our patients towards their highest level of health. Additional responsibilities include:

  • Coordinating patient flow with the receptionist and dental providers.
  • Assisting the Dental Receptionist on an as needed basis.
  • Perform procedures such as x-ray exposure and development, fluoride therapy, patient study and working models, etc.
  • Maintains the inventory and proper sterilization of all instruments and supplies, excluding medicines, and prepares rooms daily.
  • Assist dental providers in treatments, diagnostic exams, and surgical interventions.
  • Uses four-handed dental technique.
  • Taking vital signs- blood pressure, temperature, pulse, and respiration taking skills.
  • Knowledge and proper handling and mixing of dental materials, cements, etc.
  • Computer use and Knowledge.
  • Maintains CPR Certification, continuing education requirements for/and certification.

Behavioral Health

Behavioral Health Care Manager (Pediatrics)

Do you pride yourself on your ability to assist patients with navigating through their behaviors, life goals, and their health goals?

Are you interested in working with a diverse group of patients?

If so, Family Care Health Centers is looking to add two (2) Behavioral Health Care Manager (Pediatrics) to our Behavioral Health team. As a Behavioral Health Care Manager (Pediatrics) you are responsible for providing care management for patients of FCHC and for clinical supervision of non-licensed care managers. Additional responsibilities include but are not limited to:

  • Function as the BH Care Manager within the integrated primary care and behavioral health team.
  • In coordination with the BHC and Primary Care Provider (PCP), enroll eligible and interested patients into the Circle of Care project.
  • Once the patient is enrolled, track patient progress towards grant outcomes.
  • As often as clinically appropriate, face-to-face visits and telephone calls with Circle of Care patients and other members of the patient’s network over the course of their enrollment in the project (3 to 12 months).
  • Establish and maintain patient registry to monitor clinical outcomes as well as key processes of care.
  • Arrange for inpatient, partial hospitalization, intensive outpatient programs as needed.
  • Attend care team meetings and document clinical information/decision-making in the patient registry.
  • Facilitate referrals to other outpatient therapists as indicated.
  • Facilitate referrals to psychiatry as needed.
  • Bill Care management services when appropriate.

Qualifications:

  • Master’s Degree in Social Work – REQUIRED.
  • LCSW (Licensed Clinical Social Worker) – REQUIRED.
  • Minimum of two (2) years experience working in the field of social work or behavioral health care is ~ REQUIRED.

Behavioral Health Consultant (Pediatrics or Adult) - LCSW

Are you passionate about providing behavioral health care to the underserved?

If so, Family Care Health Centers (FCHC) is on the hunt for a Behavioral Health Consultant (Pediatrics or Adult) who is an LCSW.

As a BHC – LMSW, the employee is responsible for providing same day behavioral health diagnostic assessment and interventions for patients of FCHC. Responsibilities for this role include, but are not limited to:

  • Collaborating with Primary Care Providers (PCPs), BH Care Managers, and other members of the team (RNs, MAs, CHWs) in the delivery of comprehensive and integrated behavioral health care services.
  • Providing same-day behavioral health consultation for patients of FCHC as requested by the PCP, including assessment, diagnostic evaluation, and brief intervention.
  • Provides presentations to PCPs on BH topics of interest.
  • Will supervise Graduate Students in Social Work or LMSWs, if applicable.

Clinical Practice Skills:

  • Define problems accurately and use best practice to implement intervention rapidly.
  • Conduct assessments with a solid understanding of DSM-5-TR, family systems, the connection between physical and mental health, and the impact of trauma on patients’ families.

Consultation Skills:

  • Tailor PCP recommendations to the primary care setting; focus on recommendations that support the provider’s treatment of BH in primary care.
  • Conduct effective curbside consultations.
  • Employ knowledge of BH Medications to help PCPs treat complex BH conditions and substance use disorders, including consultation with psychiatry (internally, MOCPAP).

Qualifications:

  • Masters Degree in Social Work – REQUIRED.
  • Active MO state license (LCSW) – REQUIRED.
  • Minimum of two (2) years’ experience working in the field of social work or behavioral health care – PREFFERED.
  • Work experience or strong desire to work in urban community health setting – PREFERRED.

Pharmacy

Pharmacy Technician

Registered as a Licensed Pharmacy Technician in the State of Missouri?

As a pharmacy technician, you would assist the Pharmacist in providing medication to patients based on physicians’ orders and prescription requirements. Pharmacy technicians are also responsible for data entry, insurance adjudication, and inter-office communications. The pharmacy technician, in collaboration with the Chief Pharmacist, is accountable for assisting our patients towards their highest level of health. This is a full-time position within the Family Care Health Centers organization.

Working Hours:

8:30 AM – 5:00 PM

Pharmacy Technician Requirements:

· High School Diploma or GED Certificate required

· Registered as a Licensed Pharmacy Technician in the State of Missouri and in good standing required

· Pharmacy Technician experience required

· Bi-lingual skills desirable

· Computer literate a must

· Knowledge of medical terminology preferred

· Must possess exceptional customer service skills

· Ability to maintain confidentiality

· Ability to function effectively in a high energy and high paced pharmacy practice

Nutrition Services

Breastfeeding Peer Counselor (Part-time)

Breastfeeding Peer Counselors are vital to FCHC because they provide breastfeeding information and social support to pregnant and breastfeeding women and their families.

Core Responsibilities:

  • Contact pregnant and breastfeeding women to encourage and support their decision to breastfeed. Contacts may take place by telephone, on-site visits, hospital visits or home visits.
  • Provide general information on breastfeeding, infant feeding choices, and information and support for women in managing common maternal and infant breastfeeding concerns.
  • Assists with or teaches prenatal breastfeeding classes with an approved lesson plan and/or organizes breastfeeding support groups.

Family Care Health Centers provides funding for Continuing Education credits for active Breastfeeding Peer Counselors within our organization.

Qualifications:

  • High School Diploma or G.E.D equivalent ~ REQUIRED.
  • At least 3 months of personal breastfeeding experience ~ REQUIRED.
  • Counseling experience ~ PREFERRED

Community Health

Community Health Director

Are you someone who is passionate about community health engagement?

Do you pride yourself on your leadership skills and ability to advocate for patients?

If so, Family Care Health Centers is looking for a Community Health Director to lead our Community Health Workers along side the Community Health Worker Supervisor. As a Community Health Director, you would be responsible for:

  • Ensures health equity and social determinants of health are integrated across the organization by working with teams to provide community health expertise related to health care delivery, health plan initiatives, care management, data collection and analysis, advocacy and grant funding.
  • Builds and maintains relationships with key community stakeholders, including those in non-health sectors, to advance partnership opportunities and cross-sector strategies which facilitate health improvement for clients within the FCHC service area.
  • Acts as an advocate for the rights of clients and their families in the health care system and community. Assists to develop and direct the advocacy agenda for the health center in collaboration with partner entities. Evaluates the systemic impact of policies which influence local and national decisions which shape the health of members and advance health equity.
  • Provides professional public health services for clients and their families in accordance with Center policies and protocols. Makes home visits to clients based on referrals from providers and members of the multidisciplinary care team.
  • Supports and/or leads grant writing efforts that add value to the operations of FCHC.
  • Represents FCHC on a community task force, coalition, and collaborates with other groups that share priorities relative to improving the health of the community.

Qualifications:

  • Bachelor’s Degree in Social Work or related field – REQUIRED.
  • LCSW in the State of Missouri and in good standing – REQUIRED.
  • Minimum of 5+ years of leadership experience managing a team in healthcare or community-based organization which services communities living in marginalized conditions- PREFFERED.
  • Ability to prepare budgets, business cases and plans.
  • Proficient in grant writing, grants management and track record of securing federal, state or foundation grant dollars for regional or local health initiatives.
