Medical Services

Thank you for choosing Family Care Health Centers, your health care partner for life.  What that means is we are your primary health care provider from childhood through adulthood.  We provide continuity of care throughout your life.  Additionally, we offer comprehensive care, which includes medical, dental, behavioral health, vision, nutrition & WIC, pharmacy, laboratory, and specialty services such as chiropractic and HIV/AIDS counseling and testing.

This dedication to providing convenient care doesn’t mean we’re more expensive.  In fact, Family Care Health Centers provides affordable and accessible comprehensive primary health care regardless of ability to pay.  Most private insurance plans are accepted along with Medicare and Medicaid. Charges for patients without insurance are calculated on a sliding fee depending on income and family size.

Family Care Health Centers does not discriminate on the basis of race, gender, ethnicity and serves a population of all ages. To bridge communication gaps with non-English speaking patients, FCHC has interpretation services for Hispanic, Bosnian, Vietnamese, Somali as well as other languages.

Family Care Health Centers provides 24-hour patient care through providers who are on call after hours for telephone consultations and inpatient hospital care.

Pediatrics

Family Care Health Centers provides comprehensive well child and adolescent care, immunizations, integrated behavioral health and treatment of acute and chronic diseases of childhood in a medical home setting, as well as hospital in-patient newborn and sick child care.

Internal Medicine

Internists provide primary health care and hospital in-patient care to adults.  This includes check-ups, vaccines and cancer screening as well as diagnosis of disease and treatment or referral. Common long term conditions treated include diabetes, hypertension, high cholesterol, emphysema, addiction and depression.

Obstetrics/Gynecology (OB/GYN)

Physicians and nurse practitioners provide regular well-woman exams, pregnancy testing, prenatal care, delivery services, minor female surgery plus family planning consultation and education on HIV/AIDS and sexually transmitted diseases. Patients may qualify for a FREE mammogram.

Reproductive Health

Family Planning services are those related to preventing or achieving a pregnancy. Services include evaluation of health conditions which influence a person’s ability to conceive or to have a healthy birth outcome. Family Planning services at Family Care Health Centers include pregnancy testing and counseling, contraception, preconception counseling, basic infertility services, STD testing and treatment for female and male clients.

Laboratory

Family Care Health Centers provides on-site laboratory services.

Vision Care

Vision care is provided through a partnership with the University of Missouri–St. Louis College of Optometry. Primary eye care services include comprehensive examinations and eye glasses for all ages. Staff diagnose, monitor, and treat many eye conditions that can alter vision. New patients are welcome, with most insurance plans accepted. The vision department has various patient assistance programs for eligible individuals without insurance. Pharmacy Services

Pharmacy staff fills and dispenses medications, offers counseling on the use of prescription and over-the-counter medications, and advises physicians about medication therapy.  Patients may be eligible for discounted medications based upon their income through the Patient Medicine Assistance Program (PMAP). Community Health Nurses

Community Health Nurses (CHNs) are registered nurses who provide home visits to families and individuals of all ages who receive services at Family Care Health Centers. CHNs provide education and conduct nursing assessments to monitor patients’ health conditions in the community. They often assist patients with navigation of health care systems within and outside of Family Care Health Centers. CHNs answer questions and concerns on many topics, which include health, safety, child development, and quality of life issues. CHNs also participate in outreach activities including participation in health fairs and community events. CHNs are dedicated to improving the health and well-being of patients and their families.

Community Health Workers

Community Health Workers (CHWs) are frontline public health workers who complete a certificate program and are trusted members of the communities served by Family Care Health Centers. CHWs assess patients’ social determinants of health and connect them to a variety of resources, which may include housing, utility assistance, legal assistance, child safety, education, employment, health insurance, and transportation. CHWs work as advocates in the community with individuals and their families to help them meet their basic needs, build their capacity, improve health outcomes, and improve quality of life. CHWs also address the needs of larger groups to promote healthy communities.
