Family Care Health Centers (FCHC) wants to be your health care home. We are proud to be recognized by the NCQA as a Patient-Centered Medical Home (PCMH).
FCHC was created with the sole purpose of serving people who need care and were not receiving it. Fifty-seven years later, patients are still at the forefront of care. A patient-centered medical home uses a model of care focused on patients and their individual needs. Research shows that PCMHs improve quality and the patient experience. Family Care Health Centers uses a team-based approach to health care. Meaning:

- Your primary care provider is able to bring in a mental health specialist and/or a Registered Dietitian during your visit (no additional cost!).
- Your primary care provider talks with your mental health specialist, nutritional specialists, dentist, and/or optometrist regularly. Together, along with you, a patient care plan is established.

WHAT TO BRING TO YOUR APPOINTMENTS
Please bring the following items to your appointments:
- your insurance information (all insurance cards)
- an appropriate co-pay (please review the sliding fee scale for pricing estimates before your visit)
- a valid identification (ex: driver’s license, permit, passport, or state ID)
Also, please arrive 30-45 minutes before your appointment time to complete the necessary paperwork when checking-in.
