Family Care Health Centers (FCHC) has made significant strides towards improving and furthering our impact on our patients & the community by increasing access to health services, improving our quality metrics, expanding our outreach efforts, transitioning and optimizing the use of our EMR system, strengthening our workforce, and continuing to implement innovative approaches to our care delivery model, including expanding access to specialty services.
