It’s imperative to know if you have the virus so you can begin treatments as soon as possible. Fortunately, FCHC offers HIV testing and treatments.

Affordable Connectivity Program (Internet)

The Federal Communications Commission (FCC) offered the Affordable Connectivity Program, which provided savings for high-speed internet.  The program ended February 8, 2024.

Patient Portal

All patients are encouraged to use our patient portal, MyChart.  Patients can communicate with medical staff, access test results, see upcoming appointments, and request prescription refills.

If you do not have a MyChart account, you can sign-up on the portal.  You’ll need an email account and the MyChart activation code on your enrollment letter or After Visit Summary.

    Family Care Health Centers is committed to caring for the underserved and underinsured.  FCHC provides care for people of ages and in all stages of life, regardless of insurance status.  If you don’t have insurance, we offer a sliding fee scale and help with Medicaid enrollment

          If your insurance is not listed, please contact your insurance company to ask if Family Care Health Centers providers are covered in your plan.  You may also call 314-83-4CARE(2273), option 1, option 5 (Billing Department).

          Sign up for Insurance

          If you’re looking to apply for Medicaid or Marketplace (healthcare.gov) insurance, Family Care Health Centers can help.  Our Outreach & Enrollment team can submit the application for you.

            1. Call 314-937-5100.
            2. Make an appointment with our Outreach & Enrollment team.
            3. Your appointment may last between 30-40 minutes.
            4. You’ll then receive a letter within approximately 3-4 weeks with your application status.

          What should you bring to your appointment?

            1. Photo ID
            2. Social Security number
            3. Income information (wages, unemployment, social security, disability, etc. — all sources of income)
            4. Social security number and income for anyone in the household

          FOR MEDICAID PARTICIPANTS:

          Annual renews began April 1, 2023. To learn about the Missouri Department of Social Services’ benefit portal, watch this short 3.5 minute video. 

          Non-Emergency Medical Transportation (NEMT)

          Some patients’ insurance plans cover transportation to and from health care appointments.  The following insurance plans offer NEMT and patients should call to see if they are eligible.

          Affordable Connectivity Program

          The Federal Communications Commission (FCC) offered the Affordable Connectivity Program, which provided savings for high-speed internet.  The program stopped accepting applications on 2/7/24.  

          FAQs:

            • Why can I not apply for the Affordable Connectivity Program (ACP)?
                • Due to a lack of funding from Congress, the ACP stopped accepting applications.
            • When will my ACP benefit end?
                • If you applied for the ACP and signed up with an internet company before February 8, 2024, you will continue to receive your benefit for now.  An end date for the program has not been announced.
            • What if I applied for the ACP benefit before February 8, 2024, and am waiting to hear back about my application?
                • As of February 8th, applications are no longer being processed.  You may qualify for Lifeline, another FCC program offering an internet discount.  Learn more at LifelineSuppport.org.
