What do I need to do to become a patient?
Call the health center nearest you to schedule an appointment. Staff will inform you of the necessary paperwork needed to bring in. New patients are mailed out an information packet along with a reminder of the day and time prior to the scheduled appointment.
What documents are required to bring to my first visit as a new Family Care patient?
1040 & W2-forms; proof of income: paycheck stub or unemployment letter; insurance card.
What is the cost per visit?
Patients with insurance pay the co-pay of their plan due the day of the office visit. Patients without insurance are served on a sliding fee with minimum $30 ($35 for dental) co-pay due the day of service.
What insurance plans do you accept?
We except several private insurance plans along with Medicaid and Medicare. Click here to see insurance plans by service.
What hospitals do doctors have privileges?
Barnes-Jewish, Cardinal Glennon and St. Mary’s Health Center
Do you accept walk-ins?
No, Family Care does not accept walk-ins for new patients.
Do you have an OB/GYN on staff?
Yes, an OB/GYN is on staff.
Are you a free clinic?
No, Family Care Health Centers is a federally qualified health center (FQHC). Staff are board certified physicians, nurse practitioners, dentists, pharmacists, nurses, dieticians, etc. Most insurance plans are accepted. Patients without insurance are served on a sliding fee based upon income and family size.
What happens if my child is sick on a day the pediatrician is unavailable?
Family Care has a triage nurse that will assess your child’s health condition and he/she will assist you.
Do you give Hepatitis A shots?
Patients will need to schedule an appointment with their provider to discuss.
What days are HIV testing performed?

Pharmacy Frequently Asked Questions
What are the pharmacy hours?
The pharmacy is open Monday – Friday 8:30 a.m.-5:00 p.m. (Open for lunch).
Can I get my prescriptions filled at the pharmacy?
Only patients with prescriptions from a Family Care provider can have their prescriptions filled at the pharmacy.
If I am not a patient of Family Care, can I get a prescription filled?
Under federal law, staff can only fill prescriptions for Family Care patients.
Can I obtain Patient Medication Assistance Program (PMAP) medications at the pharmacy?
Yes, the pharmacy staff will assist in any way possible to lower your prescription costs through the PMAP. Pharmaceutical companies offer different PMAP plans with different eligibility criteria. Not all patients qualify for PMAP.
Does the pharmacy have samples?
No, the state board of pharmacy will not allow pharmacy staff to give out samples.
Does the pharmacy stock controlled substances or scheduled drugs (narcotics, etc.)?
No
Will I need my Medicaid or insurance card to get a prescription filled?
Yes, patients need to bring a Medicaid or insurance card to receive pharmacy services.
Are prescriptions paid for at the pharmacy counter?
Yes, full payment for prescriptions is due when received from the pharmacy staff. No prescriptions are given without payment.
Are my prescriptions free?
No. Family Care accepts most private insurance plans along with Medicare and Medicaid. Patients without insurance are served on a sliding fee based upon income and family size.
How long will I wait for my prescription?
Prescription wait times vary depending upon the number of customers being served or the number of prescriptions presently being filled.
